ABSTRACT

Objective@#To explore the relationship between nutritional status and puberty onset in boys, and to provide a reference for promoting the development of physical and mental health of boys.@*Methods@#A total of 2 724 boys aged 7 to 12 years from grade 2 to 6 were recruited from Xiamen city by cluster sampling method in 2017. The nutritional status was assessed by physical examination, pubertal developmental status was evaluated by rating scales of Tanner and Prader orchidometer, and puberty timing was determined by the P25 age of puberty onset. The association between nutritional status and puberty onset was estimated by logistic regression model.@*Results@#Pubertal onset was found in 29.0% of the boys and the incidence of early pubertal timing was 2.9%. The prevalence of puberty onset in wasting, normal weight, overweight and obesity boys was 19.6%, 28.7%, 34.4% and 31.5%, respectively. The age of puberty onset was significantly earlier in obese boys (F=3.23, P<0.05). The results of Logistic regression analysis showed that with the increase of BMI, the possibility of puberty onset and risk of early pubertal timing increased. After adjusting for confounding factors, the odds of puberty onset in boys with wasting decreased by 64.0% (OR=0.36, 95%CI=0.22-0.60), the possibility of puberty onset and risk of early pubertal timing in boys with obesity increased by 78.3% (OR=1.78, 95%CI=1.14-2.79) and 192.9% (OR=2.93, 95%CI=1.46-5.86), respectively. These relationships were more pronounced in boys of households with lower economic level (P<0.05).@*Conclusion@#BMI was positively correlated with puberty onset in boys, the odds of puberty onset and risk of early pubertal timing were significantly increased in obese boys, especially in those with low household economic level.

ABSTRACT

Objective To investigate the efficacy and safety of Qingpeng ointment treating adult chronic eczema.Methods In May 2018,cases of chronic eczema were searched in databases of Chongqing Weipu (2000-2017),China Knowledge Resource Integrated Database (2000-2017) and Wanfang (2000-2017) through Nanjing Medical University Library website in the Department of Dermatology,Suzhou Science and Technology Town Hospital.RevMan5.3 software provided by Cochrane collaboration was used for Meta-analysis.To ensure data reliability,only the documents from Grade three hospitals were included.The experimental group was treated with Qingpeng ointment,while the control group was treated with weakly potent glucocorticosteroid,such as glucocorticoid cream,or moderately potent glucocorticosteroid,such as hydrocortisone butyrate cream,fluticasone propionate,halometasone and triamcinolone urea cream.Results Totally,7 randomized controlled trials,among which 258 patients in the trial group and 242 patients in the control group were included.The odds ratio (OR) values of Qingpeng ointment relative to glucocorticoid group at 2 and 4 weeks were 0.95 (0.52,1.72) and 0.80 (0.29,2.25) respectively,showing no significant difference (P =0.54and P =0.60,respectively,95 % Confidence Interval).Compared with hydrocortisone butyrate treating group,the OR value was 0.75 (0.31,1.82).And compared with moderately potent mometasone furoate,there was no significant difference in two-week and four-week treatment groups 0.99 (0.50,1.95),1.10 (0.18,6.53) (P=0.38 and P=0.36).No obvious adverse reactions were found in Qingpeng ointment treated group.Conclusions Qingpeng ointment is safe and effective in treating chronic eczema.There is no significant difference between Qingpeng ointment treating group and glucocorticosteroid treating group.Qingpeng ointment can be used as a safe external medicine for the treatment of chronic eczema.

ABSTRACT

Objective Psoriasis vulgaris (PV) is easy to prone to recur and hard to cure and little research has been done on combined treatment on PV.The article was to study the clinical effects of total glucosides of paeony capsules (TGP) combined with acitretin and compound flumethasone on PV as well as the peripheral blood cytokine levels.Methods 126 patients with PV who visited our hospital from October 2015 to January 2017 were randomly divided into combined treatment group (63 cases) and control group (63 cases).Both groups were treated with oral acitretin and topical compound flumethasone, what's more, the compound flumethasone group received oral TGP treatment, 8 weeks for a course.The clinical therapeutic effects were evaluated by the levels of peripheral blood IL-17, IL-18, IL-23, TNF-α level, PASI score and percentage of total skin lesions before and after the treatment.Results After the treatment, the concentration of IL-17, IL-18, IL-23 significantly decreased(P<0.05), which was significantly less in combined treatment group compared with control group (IL-17 [61.18±8.91] vs [78.64±7.85], IL-18 [68.56±17.95] vs [79.49±18.64], IL-23 [70.13±12.16] vs [91.18±16.89] pg/ML)(P<0.05).Moreover, the TNF-α level, the PASI score and the percentage of total skin lesions significantly decreased in both groups after treatment(P<0.05), which was significantly less in combined treatment group compared with control group (TNF-α level [14.47±7.53] vs [23.49±8.12]ng/L, PASI score [4.09±1.29] vs [7.29±5.13], the percentage of total skin lesions [6.17±4.59]% vs [8.09±5.18]%) (P<0.05).Conclusion TGP combined with acitretin and compound flumethasone can significantly enhance the clinical therapeutic effects and effectively regulate the levels of the IL-17, IL-18, IL-23 and TNF-α level, which results in treating psoriasis vulgaris.

ABSTRACT

An 18-year-old woman was admitted to the hospital for swelling of the right lower limb and vulva with milky-white vesicles for 3 years.Skin examination revealed swollen and enlarged right lower limb and labium,numerous clustered or scattered pinhead-to millet-sized whitish thick-walled vesicles over the right labium majus,bilateral labium minus,vaginal orifice,and right thigh,with milky-white chylous fluid draining from the vagina.Magnetic resonance imaging revealed obvious lymphangiectasia in the lower abdominal cavity and right side of the pelvis,dilation and distortion of lymphatic vessels in the skin and subcutaneous tissue of the right inner thigh,the vaginal wall and at the medial side of the right labium majus,but no abnormality in the uterus.Histopathological examination of the milky-white vesicles showed cystic dilation of lymphatic vessels in the superficial dermis,and fibrous hyperplasia of the middle and lower dermis.A diagnosis of primary chylous reflux syndrome Ⅰ was made.

ABSTRACT

A case of porokeratoma is reported.A 35-year-old man presented with asymptomatic verrucous nodules on both buttocks for more than 1 year.Skin examination at the first visit revealed two pea-sized verrucous nodules on bilateral buttocks.The lesions recurred at the primary site 3 months after surgical resection.At the second visit,there were 4 verrucous nodules,including 1 on the left buttock and 3 on the right buttock.Histopathological examination showed a well-defined lesion characterized by acanthosis and compact,thick orthokeratosis with seveal broad cornoid lamellae.A diagnosis of porokeratoma was made.The lesion was resected again followed by oral acitretin treatment.No recurrence was observed during 15 months of follow up.

ABSTRACT

Nine mutants (P2M1-9) were obtained using PCR with 5-BU based on DNA template (P2Y) encoding the active region of Parasporin-2. Mutant proteins were purified after expressing in E. coli BL21 cells, followed by assayed against hepatoma cells and normal liver cells by MTT. They showed diverse anti-hepatoma activities, in which two mutant proteins, P2M1 and P2M8, exhibited high cytotoxicity against hepatoma cell lines SMMC7721 and Be17402, meanwhile leaving normal liver cells Chang-liver unaffected. Structural comparison among P2Y, P2M1 and P2M8 showed that the length of beta-sheet or beta-fold, and the amount of alpha helix greatly affected the anti-hepatoma activity of Parasporin-2. Results based on amino acid alignment, molecular docking between P2Y, P2M1 or P2M8 and receptor, and mimic mutation demonstrated that amino acid residues at the sites of 52, 56, 58 and 208 on P2Y, especially the aromatic amino acids such as Trp, Phe, and Tyr were involved in the interactions.

ABSTRACT

Objective To assess pathological features of muscles as well as microvascular changes between muscle fibers in patients with dermatomyositis (DM),and to analyze pathological differences in muscles between DM and polymyositis (PM).Methods Specimens were obtained from involved muscles of 16 patients with DM,5 patients with PM,and from normal muscles of 9 patients with bone trauma (controls).Routine histopathological examination and immunohistochemical staining for CD34 and CD61 were conducted.Results Of the 16 patients with DM,6 (37.5%) had perifascicular atrophy,3 ( 18.8 %) had an obvious inflammatory cell infiltration around microvessels between muscle fibers.Perifascicular atrophy was absent in muscle specimens from patients with PM or bone trauma.The number of CD34-positive microvessels between muscle fibers was reduced in patients with DM,but normal in those with PM.CD61 was positive in perifascicular area of 10 patients (5 moderately positive and 5 weakly positive) with DM,with an expression rate of 62.5%,however,only 1 case of PM was weakly positive for CD61,and all the controls were negative.Conclusions There is a decrease in the number of microvessels but an enhancement of neovascularization between muscle fibers in involved muscles of patients with DM,which may serve as a pathological marker to distinguish DM from PM.

ABSTRACT

Objective To investigate the operative methods and their efficacy of the modified decompressive craniectomy in combination with indirect revascularization for the treatment of massive cerebral infarction. Methods The clinical data of 15 patients with massive cerebral infarction who underwent modified decompressive craniectomy in combination with encephaloduroarteriomyosynangiosis were analyzed retrospectively. The clinical efficacy was assessed according to the mortality, Barthel Index and modified Rankin scale. Results All the patients were followed up, and 12 patients survived 3 months after operation, of those, 4 had a good functional outcome; 11 patients survived 6 month after operation, of those, 7 had a good functional outcome. Conclusions The modified decompressive craniectomy in combination with indirect revascularization for the treatment of massive cerebral infarction could effectively treat massive cerebral infarction and increase the survival rate and life quality of patients, however, the surgical timing, surgical indications and surgical skills are needed to master accurately.

ABSTRACT

Objective:To study the best opportunity for minimal invasive puncture in the treatment of hypertensive intracerebral hemorrhage.Methods:Forty-one patients with hypertensive intracerebral hemorrhage were randomly divided into four groups:<6 h,6-12 h,12-18 h and 18-24 h from the onset of symptoms.The comparison among groups in outcome measures were performed,including the rebleeding rate,short-term efficacy,long-term efficacy,mortality,and excellent and good rate.Results:The incidence of complication such as rebleeding etc had no significantly difference in the four groups.The excellent and good rates of clinical outcomes at 3 months were the<6 h group>6-12 h group,and 12-18 h group>18-24 h group(P<0.05).The 18-24 h group had the highest mortality(30%,3/10).Conclusions:Within 6 h after the onset of hypertensive intracerebral hemorrhage was the best opportunity for the minimal invasive puncture.

ABSTRACT

Objective To explore the musculoskeletal MR findings in dermatomyositis and to evaluate the role of MR in diagnosis of dermatomyositis. Methods Twenty patients with early dermatomyositis and 25 patients with active disease confirmed by pathology underwent T 1-weighted (SE 450/20), T 2-weighted (FSE 4000/100), and fat-suppressed MR images. MR findings in these 45 patients were analyzed. Results Within all the myositis, MR showed symmetrical slight high patchy signals on T 2-weighted and STIR images in the muscles of bilateral thighs, but the morphology of muscles appeared normal in 26 patients, while limited high signals only appeared on STIR image in 19 patients. There was correlation between serum muscle enzymes and the signal intensity as well as affected field of myositis. The myositis often affected gluteus and the external lateral muscles in the lower part of thighs. Inflammatory changes in the subcutaneous fat in 18 patients with active dermatomyositis were the severe appearance of the disease. Inflammatory fascia in 17 patients was found in both early and active cases. 5 patients had skin damage. Conclusion MR features of dermatomyositis are multiple myositis, fascia and subcutaneous inflammation, with the latter being an appearance of severe period of the disease. Early affected location of dermatomyositis is gluteus and the external lateral muscles in the lower part of thighs. MR is an efficient examination and STIR is a sensitive sequence in the diagnosis of dermatomyositis.
